You can request a packet of information, including forms for filing applications, from the U.S. Patent and Trademark Office. To do so, you can call 800-786-9199 and ask for the Inventors Assistance Center, which is available from 8:30 a.m. to 5 p.m. Eastern Time, Monday through Friday. Or you can visit www.uspto.gov/go/forms. However, not all forms can be filed online.

When applying for a patent, you can apply for a non-provisional application, which starts the process and may lead to a patent, or you can file a provisional application, which establishes a filing date but does not begin the examination process. The USPTO says you can file both types of patent applications either electronically by using the Electronic Filing System http://www.uspto.gov/ebc/efs/index.html or in writing to the Commissioner for Patents.
