First check with your company to find out whether any standard employee appraisal forms exist. Many larger companies prefer to have all supervisors use the same or similar review forms across the board, and often they require managers to evaluate employees at a certain time each year. Even if your company doesn’t require this, they may have sample evaluation forms on hand for your use.

For a generic performance appraisal form, check online. Many companies offer free standard appraisal forms online that you can download. Other companies offer standard or customized forms for a fee, and these fees range greatly.

You can create your own forms, but it’s best that you find a sample form, maybe borrowed from a company you admire or use a free one found online, to serve as the backbone of your form. Then change the wording to address your specific company. However, make sure the forms are worded fairly, giving employees and you a chance to answer honestly and completely. Don’t railroad employees into saying positive things, when they really need to “let it all out.”

To be effective and legal, appraisals must be job-related, based on a thorough analysis of the job, standardized, not biased, and performed by a manager with adequate knowledge of the employee or job. The best appraisal systems have a system in place to deal with the event that an employee disagrees with his or her supervisor or feels that he or she has been dealt with unfairly.
